The Look Cosmetic Group provides prescription and supply services exclusively to qualified health professionals. To ensure compliance with regulatory standards, all practitioners must complete an onboarding process and be assessed for eligibility before gaining access to these services.
By submitting your details through our secure onboarding form, we can verify your credentials and support you in accessing prescription pathways, supply ordering, and practitioner resources.

Our Process
Information for obtaining a script and administering aesthetic treatment

Please have the following ready:
Valid AHPRA registration
Current CPR or BLS certificate
Clear photo ID (driver’s license or passport)
Relevant training and certification in aesthetic injectables
(If you have no prior injectables training, see Step 3 for further details)
Current professional insurance certificate

Fill in your details and upload your documents via the secure online form below. This allows us to review your credentials and begin the onboarding process.

An assessment is required for all new injectors when opening an account.
If you do not have prior injectables training, you will be required to complete a training course and an assessment with Dr. Cindy before prescribing approval.
You will be contacted to arrange a suitable time for your assessment.

Once your documents have been reviewed and your assessment is complete, your account will be approved and activated. You’ll receive an email confirmation and further instructions for submitting prescriptions and ordering products.
